Membership Program

Membership entails paying a one-time enrollment fee plus a lump sum yearly (or monthly) subscription fee to Proactive, in return for medical services exclusively through Proactive Ambulatory, up to an annual maximum limit.

The Proactive Medicare Program is a multilevel membership plan with various AFFORDABLE plans offering differing levels of benefits, each with a preset annual limit.

Membership in the Proactive Medicare Program entitles the subscriber to basic services:

• Access to consultations with Physicians

• Laboratory Services

• Radiological Services (x-rays, ultrasounds)

• Discounts on prescription medication

• Access to emergency care

• Inpatient Services

Subscribers with Health Insurance can use this program in conjunction with their Health Plan by affording the insured subscriber the opportunity to assign benefits of any balances in excess of that covered and items or services that may not be included in this Medicare program.

Subscription to this program does NOT mean that the member is obligated to see any physician associated with Proactive unless they choose to do so. However, coverage of doctors fees are only for Proactive doctors.

Proative Medicare Plus

An outpatient service only program offering an annual maximum benefit of $2000.00 in OUTPATIENT services.

$45 per month with a one time enrollment fee of $50 and $25 for children under 16 on family plans.

Single parent plan: Parent pays $45 per month with one child free. $20 per child thereafter.

Family plan: Principal and spouse pay $45 each, one child free and each child at $20 per month thereafter.

Proactive Medicare Silver

An inpatient service only (hospitalization) program offering an annual maximum of $3500.00 in services.

$55 per month with a $50 one time enrollment fee. $25 for children under 16 on family plans.

Single parent plan: Parent pays $55 per month with one child free. $20 per child thereafter.

Family plan: Principal $55 and spouse $55 each, two children free and each child at $20 per month thereafter.

Proactive Medicare Gold

Our most comprehensive plan that offers inpatient as well as outpatient coverage to a total of $6000.00 annually.

$90 per month with a $50 one time enrollment fee. $25 for children under 16 on family plans.

Single parent plan: Parent pays $90 per month with one child free. $20 per child thereafter.

Family plan: Principal $90 and spouse $90 each, three children free and each child at $20 per month thereafter.
